Access Care Plus is a newly established primary care office dedicated to providing diligent care to our patients. Access Care Plus is an extension to Access Adult Health Day Care Center, which was established in 2011, to provide care that elongates and improves our patient’s quality of life.

Our goal is to provide timely medical care in a friendly patient-centered environment. We pride ourselves on the ability to offer same day appointments for most symptoms. During your visit with Access Care Plus, you’ll receive health and wellness care from an experienced Nurse practitioner which blends the intensely personal aspect of time-honored healthcare with all the advancements and benefits of modern care.
